For Patients

We request that you arrive 15 minutes prior to your scheduled appointment time in order to allow our staff to process your insurance and medical information prior to your visit with the doctor. If you have had previous laboratory testing, ultrasounds, etc., it is imperative to bring these results to your visit to assist your physician in providing you with the best diagnostic and treatment plan. If you do not have access to your records then please let our office staff know in advance so we can attempt to have all the relevant medical information available for your consultation.

Please note, if you are unable to keep your appointment please give us at least 24-48 hours notice. This courtesy allows us to better be of service to other patients.
